Sweet Medical Center Inc., established in 1988, is a Community Health Center and provides medical services regardless of ability to pay. Please call (406) 357-2294 for an appointment. All donations are welcome and tax deductible.
If you are coming to our clinic as a new patient, you can save yourself some time by printing the New Patient Forms, filling them out at home and bringing them with you.
If you need us to send your records to another provider, you may print this Release of Information Form, fill it in and mail/fax it to us.
