What is the Health Assistance Program (HAP)?

Sweet Medical Center offers the Health Assistance Program or (HAP) to individuals who have a total household income that is less than 200% of the current poverty level.

The Sliding Fee Scale is a formula used by Sweet Medical Center to determine if you qualify for assistance.

WHAT DOES HAP ASSIST YOU WITH?

  • REDUCED CHARGES FOR MEDICAL SERVICES AT SWEET MEDICAL CENTER

The discount applied to your medical charges will be determined based on Household Income and Family Size in comparison to Federal Poverty Income Guideline (FPIG).  Discounts will apply to lab work done in house.  Lab work sent out to our 3rd party lab may be billed to you.  We ask that you bring in those charges so we can apply the reduced fee.

  • DENTAL ASSISTANCE

First, you must make an appointment with a Sweet Medical Center medical provider.  If our provider refers you for dental care, SMC will pay up to $250 per patient per year for exams, cleanings, fillings, and extractions.  You will make all of your arrangements through our HAP office, and they will work with you to set up an appointment with a dentist. You will need to pay a $15 co-pay for each dental visit.

  • VISION ASSISTANCE

First, you make an appointment with a Sweet Medical Center medical provider.  If the provider refers you for vision care, SMC will pay up to $180 per patient per year for treatment eye exams and glasses.   You will make all of your arrangements through our HAP office, and they will work with you to set up an appointment with Dr. Obie in Chinook. You will need to pay a $15 co-pay for each visit. Yearly eye exams are not covered under this assistance.  Funding is limited and services are provided on a first come first serve basis. Certain criteria may apply.

  • MENTAL HEALTH ASSISTANCE

First, you make an appointment with a Sweet Medical Center medical provider. If the provider feels you need mental health services, we will pay for 8 visits to the Center for Mental Health.  There is an option for continued services.  You will make all of your arrangements through our HAP office, and they will work with you to set up your appointments.  You will need to pay a $15 co-pay with each mental health visit.

Depending on which category you qualify for on the Sliding Fee Scale, the following assistance may also be available to you:

  • Pharmacy Assistance

$12 voucher(s) available every month to assist with medication expenses and postal assistance with mail order pharmaceuticals.

  • Transportation Assistance

$10 voucher available every month to assist with fuel expense.

 

Program Overview and Information Sheet

HOW DO I APPLY FOR THE HEALTH ASSISTANCE PROGRAM?

  1. View the HAP Sliding Fee Scale.
  2. Complete the HAP application.
  3. Fill it out and send/bring it in with proof of income.

Proof of income can be the first page of your most recent income tax statement - we use the Adjusted Gross Income at the bottom right of the page. Or you can use your last three pay stubs, your last three bank statements, or your annual Social Security Statement.

* Your HAP enrollment covers all family members in your household for one year from the application date and must be renewed annually.

HOW TO READ THE INCOME CHART:

  • Level A: A $15 nominal fee is due at each visit. This co-pay covers all medical services at SMC. It does not cover the interpretation of x-rays at Northern Montana Hospital, who will bill you separately.
  • Level B: A $25 co-pay is due at each visit. This $25 co-pay covers all medical services at SMC. It does not cover the interpretation of x-rays at Northern Montana Hospital, who will bill you separately.
  • Level C: A $35 co-pay is due at each visit. This $35 co-pay covers all medical services at SMC. It does not cover the interpretation of x-rays at Northern Montana Hospital, who will bill you separately.
  • Level D: A $45 co-pay is due at each visit. This $45 co-pay covers all medical services at SMC. It does not cover the interpretation of x-rays at Northern Montana Hospital, who will bill you separately.
  • Level E: A $55 co-pay is due at each visit. This $55 co-pay covers all medical services at SMC. It does not cover the interpretation of x-rays at Northern Montana Hospital, who will bill you separately.


If you have health insurance but meet the income guidelines for the HAP, we will bill your insurance first, and discount the remaining unpaid charges according to the HAP income chart.
